A musty, earthy odor in a bathroom that does not go away with cleaning is microbial volatile organic compounds (MVOCs) released by active mold colonies. The smell itself is not dangerous, but it tells you mold is alive and feeding somewhere in or near the room, and that exposure to mold spores is ongoing.
